Oversize/Overweight Permit Revision Request Form

MnDOT will consider revision requests on a case-by-case basis.

Account holders with a valid username and password may submit a one-time revision request online to update truck and trailer information only.

Use this form to submit your revision request if:

You don’t have a username and password; or,
You need to change any of the original overall dimensions listed on the permit

Note: MnDOT will not grant a revision when any of the following exist:

There is an increase in the overall GVW;
There is an increase or change in axle weights;
The original route changes; and/or,
A bridge analysis review is required
